One quibble:

If you went on the Forest School, what did you think of it? *

That caught me out as it seemed you only needed to answer it if you  
went on it, whereas actually it was a required field.

(But then I don't know when I last filled in a web form that didn't  
get chucked back to me. At least this one didn't have password inputs  
and captchas that must be life-drainingly re-entered each time round ...

OK I should shut up moaning or become a UX engineer.)
