There are a few approaches to take, the one I know about is to use
jwplayer.

Grab a copy of your video, get jwplayer installed and pointed to the
video. Then you can specify multiple caption files which are typically
text files stored on your webserver (srt files or the new html5 format
which I can't remember the name of).

You can let people embed your jwplayer from your server with an iframe
(same as how vimeo do it).
