[HacktionLab] survey Qs

Hi guys!

Usually best think to do is implement Qs in a form (which possibly 
offers/includes data analysis tools) and run it as a pilot with one or 
two tester(s) before spreading final version.

I've got some experience in the field and I'd be happy to give an hand.
