[SSC] Sign up form prep

hi all,

I've set up a google form as the basis of our sign up on the website (to be ready for when the letter to academics goes out) - I think the plan will be to have a form and an embedded spreadsheet which updates in realtime when somebody has signed up (but need to think carefully about how much we share on the public document) I've take the values for the form from the member's document - which is really basic information as you can see. I think as we expand we might want to consider more details to help with organising activities at a later date - I will leave this open for discussion (it can always be changed!)
